BRONCHIAL TREE

ORGANIZATION OF THE BRONCHIAL TREE

The trachea bifurcates into the left and right primary bronchi, which supply the left and right lung, respectively. Each lung is organized around its system of airways, referred to metaphorically as the bronchial tree, in which the primary bronchus is the trunk and the smaller bronchi, bronchioles, and respiratory airways are its branches. The blood supply to the lungs closely follows the branching pattern of airways in the bronchial tree. Each branch of the bronchial tree ventilates a distinct part of the lung.

The left and right primary bronchi enter the lungs and bifurcate (left) or trifucate (right) into secondary (lobar) bronchi to supply the two lobes of the left and three lobes of the right lung, respectively. Each secondary bronchus branches dichotomously (up to 16 times), producing progressively smaller airways. The larger airways are classified as bronchi; smaller airways (<5 mm) are called bronchioles (Figure 10-2A). The most distal conducting airways are the terminal bronchioles. Terminal bronchioles branch to give rise to respiratory bronchioles, the initial airways in the respiratory portion of the bronchial tree.
